Sporting Licence Application for Minors (below 18 years of age)

Please follow the steps below:

1. Download this form : Sporting Licence Application Form for Minors
2. Fill the form, scan and send it to us by email at sec@afs.org.sg
3. Send us the hardcopy of the application by post to : 100 Haig Road, Dunman View, #10-02, Singapore 438797
4. In the hardcopy submission, to include  (i) a letter declaring the Singapore residency status of the child, whether he or she is SC/PR/DP/LTVP [signed by parent/guardian with date] and (ii) passport photo
5. Your licence can be delayed if we don’t receive all of the documents (both email as well as hardcopy documents)
