Sporting Licence Application for adults 18 years and above, please follow the steps below:

1. Download this form : Sporting Licence Application Form
2. Fill the form, scan, and email it to us at sec@afs.org.sg. Please ensure all signature fields are endorsed.
3. In the email submission, please include  (i) a scan of your IC (you may edit out the IC number except last 4 characters) and (ii) passport photo
4. A follow up email reply from us will include details of fees to be paid. Upon fee payment, the Sporting Licence will be issued.
5. Your licence can be delayed if we don’t receive all of the documents (both online documents as well as hardcopy documents)
